Karlach’s infernal engine keeps running hot, and it needs to be fixed. But who can perform the maintenance, and what materials will they need? This guide gives you all of the answers.
Background
After defeating the “Paladin’s of Tyr” in the tollhouse on the Risen Road, if Karlach is with you then she’ll gain the condition Unbridled Wrath, which will cause her to burst into flame and start destroying everything around her. Afterwards, she’ll inform you that she has an infernal engine instead of a heart, but that it’s been overheating lately, causing outbursts like the one you just saw, and might require a tune-up.
Luckily, Karlach will know that there’s supposed to be an infernal mechanic in the area — a Tiefling like herself — and so you’ll decide to go to the Emerald Grove since that’s where all the Tieflings are. If you met Dammon previously, then you’ll know he’s the one you’re looking for. Meeting Dammon will trigger the quest.
Walkthrough
When you talk to Dammon, he’ll concur with Karlach‘s assessment, but he won’t be able to do anything to help her without Infernal Iron. Fortunately, he’ll inform you that he can sense some of that material in the area, and he’ll mark the blacksmith’s house in the Blighted Village on your map. Once there, you’ll find multiple entrances into the Whispering Depths.
In the forge part of the Whispering Depths, you’ll discover a wooden chest above a ladder. The chest will be locked. To open it, you’ll need to pass a DC-15 Sleight of Hand check to pick its lock, or you’ll need to pick up its Heavy Key, which is located just outside the blacksmith’s house to the northeast. Inside the chest, you’ll find Infernal Iron plus a Steelforged Sword.

When you return to Dammon, he’ll hammer on the Infernal Iron for a bit, and then he’ll hand it off to Karlach, who will use it to patch up her infernal engine. Karlach will be happy with the result, and she’ll gain the Heart of Iron feature (which will give her Advantage on Saving Throws against being Charmed), but Dammon will note that the engine is still running hot, and he’ll ask for some time to come up with a solution. In the meantime, he’ll suggest that you find more Infernal Iron.
Along with the Infernal Iron marked by Dammon, you can find three other infernal samples during Act 1:

But you won’t be able to do anything with these extra samples until you meet with Dammon in Act 2.
Act 2
In Act 2, you’ll find Dammon in the stables of the Last Light Inn. When you talk to him there, he’ll tell you that he has some good news and some bad news:
The good news is that with one more piece of Infernal Iron, Dammon will be able to contain Karlach‘s heart enough that she’ll be able to touch people again — which might be good news for you too if you’re romancing her. This will upgrade Karlach‘s Heart of Fire feature to Heart Ablaze (which will prevent her from being Charmed entirely).
But the bad news is that Dammon won’t be able to contain Karlach‘s heart completely, and if she remains in the Material Plane, then eventually her heart will explode. The only way for her to survive is to return to Avernus — permanently. Karlach, of course, will decide that she’d rather stay here than go back to the Nine Hells and become Zariel’s puppet again.
The quest will end when you give Dammon a sample of Infernal Iron. For any extra samples you find, you can have Dammon make Flawed Helldusk Armour, Flawed Helldusk Gloves, and a Flawed Helldusk Helmet — one sample for each. There are 11 samples in the game, but you only need five of them for this quest plus the armor pieces. You can sell the rest.
