You get it under a tent, in the area past Mohg, the Omen in the Subterranean Shunning-Grounds. This area is accessible after defeating Morgott, the Omen King in Leyndell, Royal Capital. You’ll have to drag a nearby enemy and bait their frenzied attack to break the nearby tent to be able to grab the note.
